Please contact our office at (407) 891-1101 or feel free to stop by our office during regular business hours to schedule an appointment. We may be able to see you within a matter of days depending on the nature of the situation. New patients can complete our New Patient Paperwork by downloading the forms here and bringing them with you to your first appointment. Insurance We try to accept all major insurance providers and Medicare. We suggest that you check with us at the time you schedule your first appointment. If your policy requires a referral from your primary physician, please make sure it is available to us at the time of your first appointment. Emergency Appointments We leave additional appointment times open during the week for emergency visits. If you need an immediate appointment, there is a good chance we can fit you in. Depending on your flexibility of schedule, we may be able to accommodate you same day or within 24 hours. Canceling Appointments We understand that patients will need to cancel appointments due to a variety of situations. Therefore, we kindly ask that you give us at least 24 hours notice and reschedule as soon as possible. By allowing us to free up your appointment ahead of time, you are helping another patient that needs to be seen sooner. Additionally, if you are running late for an appointment, please contact our office to let us know. |
